Giving birth in the UK is getting more dangerous; the number of women dying in childbirth is up 20% in the last ten years. Now, in Nottingham, the largest-ever maternity inquiry is underway, reviewing the medical records of thousands of births for signs of avoidable harm.

Reporting for Panorama, Michael Buchanan hears powerful testimonies: from mothers who weren’t listened to, staff who blew the whistle but were ignored, and from parents whose babies’ tragic deaths were dismissed as ‘just bad luck’.
