Did a Trident missile test fail?

When MPs voted last summer to spend 拢40 billion replacing the submarines that carry our nuclear weapons, they were not told that just a few weeks before, something went wrong when one of those submarines test-fired a nuclear missile. What happened and what did Theresa May know about it?
