Twenty-five year old Dean Saunders died after he electrocuted himself at Chelmsford Prison in Essex in January 2016. An inquest and the Prisons Ombudsman found that Mr Saunders, who had been briefly held in a mental health secure unit, had been let down by the justice system. His parents and partner have been giving evidence to MPs and Peers investigating deaths in prison, as Rachel Byrne reports.
